a good sized spider is now an ideal mount, if you can tame it before it eats you, rats are now the size of elephants (hungry, meat eating, elephants), and tell me, does your fire ball spell still impact the same area, or has its range shrunk as well?

Genticly modified humans with enhanced powers (the G-Mods) are possible; but Gene-corps Inc. keep them tightly controlled by only creating G-Mods that are utterly dependent on the amino acid 'soup' that Gene-corp also manufacture
-A wizard stumbles into the local tavern asking for help and collapses before the PC's. He is found to be infected with a plague that ravaged these lands generations ago. The only cure requires a supply of a certain herb. But the only place nearby where enough of it can be found is a trackless forest crawling with giant spiders.
-In an urban setting: Murders are occurring with alarming frequency. Once the PC's become involved, they learn from the local thieves guild that the crime scenes resemble those of an infamous mass murderer who went on a killing spree 80 years ago, but that the authorities are keeping that quiet. The collected writings of the original mass murderer have gone missing. It turns out that what everyone assumed to be the cryptic ramblings of a madman were actually the bloody teachings of a dark prophet...
-The PC's are on a ship that is taken by pirates near the coast of an unexplored land of dark jungles. They manage to escape and make it to shore. They must befriend a peaceful (but distrustful) tribe of natives by retrieving a sacred and powerful relic from the ancient temple of their ancestors. Only then will the peaceful tribe be able to defeat the nearby hostile tribe who trade with the pirates. And only then will the PC's be able to free the captive crew and take back the ship to resume their voyage.

Here's one that happened to me in real life. A man walks by, his face contorted in fury as he speaks into a cell phone, "I tried out that arsenic you sent me last night and it didn't do a THING for me! Are you going to send a fresh shipment tonight or am I going to have to get rough on you?!"
After some consideration, I followed up on it and discovered the situation was harmless but I have used it several times in different genres.

It's got the party guessing and me realizing that, while I have a ton of NPCs generated, on precious few did I document their eye colour.

heh, I love that idea! reminds me of a short story (by China Mellville in "looking for Jake" perhaps? I cant recall exactly who it was by) in which entire streets would appear and disappear, and the vast majority of people didn't even notice anything strange ... but they weren't actually streets, they were some sort of creature that would consume anyone who walked down them!
A well-liked NPC comes to the PCs with a sob story. Their child, wanting to emulate their heroes (the PCs), played Adventurer and snuck into the temple of an evil-aligned deity in the city last night. No one has seen them since.

The head of an unknown man has been found on the stoop of the inn where the adventurers spent last night. No one recognizes the person or saw how the head came to rest there. Bonus points for using a demi-human race such as a goblin or kobold whose kind isn't native to the area and hasn't been seen in a long time.
no idea what to do with this... but there is the seed of a game in here Im sure (playing the grizzled, weather beaten 'Monkeys' who animate at night and defend their town perhaps?
